PIAA CIPPE Form – Sections 6-7
– Section 6 – completed by Parent/Guardian
– Section 7 – Must be completed by doctor/physician
– Can be uploaded to your Family ID registration
PIAA Recertification Form – Section 8 & 9
– Must be completed for any subsequent sports within the same school year.
- Example – A student-athlete plays a Fall sport as well as a Winter/Spring sport. This student would need PIAA CIPPE Sections 6-7 (above) for the Fall Sport. This student would only PIAA CIPPE Section 8 for their Winter/Spring sport.
– Section 8 can be completed online at Family ID
– Section 9 only needs to be completed if you answered “Yes” to any of the questions on Section 8. If all answer are “No”, Section 9 does NOT need to be completed.
